In order to redirect 2% from salary tax to our organization, please download the precompleted form from here (Romanian), then print it and complete only chapter I: "Date de identificare a contribuabilului", then directly down, signature and date ("Suma" from chapter III, point 2 remains unfilled).
 
Hand it to your local financial administration until 25 May 2012.
